Appeal Form

This form is to be completed by the student and is designed to accompany the appropriate form (e.g., Application for Graduation, Withdrawal Form, etc.) per your situation to document your reason(s) for not adhering to the posted deadlines or policies.

Class Scheduling Form

This form is to be used by the student to register for classes in-person. Upon completion, please submit to the Registrar's Office for processing.

Confidentiality Request Form

Confidentiality Request Form

This form is to be used by a student to request that directory information be withheld from public inquiry. This form should be submitted if the student desires to restrict access to directory information from any entity that is NOT the student (including a parent, spouse, etc.)

Course Drop Form

Course Drop Form

This form is to be submitted by a current student to drop a course or courses from their schedule after the published census date. Please note: If you want to drop ALL courses from your schedule, you will need to submit a Withdrawal Authorization Form.

This form is to be used to certify a current student's enrollment and/or obtain a letter of good academic standing.

FERPA Waiver Form

FERPA Waiver Form

This form allows a student to grant access to their education records to other entities besides themself, such as a parent, spouse, etc. This request will remain in effect until revoked by the student.

This form is to be used by a current student to cancel their Application for Graduation.

Pass/No Pass Declaration Form

Pass/No Pass Declaration Form

This form allows the student to elect that a course be entered as a Pass/No Pass option to their academic record. Please review the Pass/No Pass Policy prior to submitting this form.

This form is to be used by a current student to request to be reinstated following their withdrawal from the University.

Request for Tuition Rebate Form

Request for Tuition Rebate Form

This form allows an impending graduate (undergraduate only) to apply for a potential $1,000 tuition rebate awardable upon graduation.

Withdrawal Authorization Form

Withdrawal Authorization Form

This form is to be used by a current student to obtain pre-authorization and advisement prior to withdrawing from all courses for a given semester. Once completed and returned to the University Registrar’s Office you will be given the Withdrawal Form PART B to complete your withdrawal.
